Community Integration Services assist, empower, and build upon the strengths of the participant and family through intensive case management to set the occasion for the participant to be fully integrated into the community with their family. Community integration services benefit families with children or youth who have multiple and complex mental health and/or behavioral concerns, and are involved in multiple services and service systems, by providing intensive case coordination.
Our team of experienced professionals provide the support that is necessary for the participant and family to move seamlessly through all community environments, including but not limited to: collaboration with outside professionals, communicating with community supports to vet appropriate supports/services for the client, and navigating alongside the parent/caregiver to identify the most appropriate resources.
This service is coordinated by a state licensed social worker and BCBA using a team approach to meet client and family needs expeditiously, and it is typically provided virtually. Forward Integrated Services establishes targeted goals at the onset of Community Integration Services, all determined to increase integration in the participant’s community. Using a team approach, we are able to provide information and support on a variety of goals simultaneously.
