Community Integration Services
Community Integration Services assist, empower, and build upon the strengths of the participant and family through intensive case management to set the occasion for the participant to be fully integrated into the community with their family. Community integration services benefit families with children or youth who have multiple and complex mental health and/or behavioral concerns and are involved in multiple services and service systems.
Our team of experienced professionals provide the support necessary for the participant and family to move seamlessly through all community environments, including but not limited to: collaboration with outside professionals, communicating with community supports to vet appropriate supports/services for the client, and navigating alongside the parent/caregiver to identify the most appropriate resources.
This service is coordinated by a state licensed social worker and BCBA using a team approach to meet client and family needs, and it is provided virtually.
